Do I need a physician referral to receive physical therapy?
You may receive a screening or examination without a referral however, prior to initiating treatment you will need to get a referral from your healthcare provider. A referral is often required prior to treatment in order for your provider to determine if diagnostic studies are necessary prior to treatment or if prescribed medications are needed to compliment the physical therapy plan. Also, insurance may not cover physical therapy costs without an appropriate referral. You can request physical therapy from your healthcare provider and generally there is no problem in receiving a referral.
Can I choose where I want to have therapy?
You have the right to choose which physical therapy clinic and clinician you want to see. Our patients come to SM&PT by physician recommendation and because of our dedication to patient care and our strong reputation within the community.
How do I make an appointment?
Once your physician makes a recommendation that you need physical therapy, call one of our five clinics and we will get you scheduled for your first appointment right away. Click here for our locations.
Does SM&PT accept insurance?
We accept most insurance plans including but not limited to: Medicare, Aetna, UHC, Humana, Blue Cross Blue Shield, most Medicaid plans, and most Workers Compensation plans. Please call before your first visit to confirm your specific insurance benefits, or if you would like to verify our participation with your health care plan.
Does your facility submit claims to my insurance company?
Yes, SM&PT will file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.
What will happen during my first visit?
During your first appointment, your therapist will conduct a comprehensive evaluation and discuss your plan of care, including the frequency and duration of treatment suggested to reach your goals. The evaluation is necessary to create a program designed specifically for you and to facilitate a maximum recovery. Each visit, you will receive one-on-one care by a therapist who is fully engaged in helping you attain your goals.
Do I need to bring anything with me?
Please bring the following with you to your first visit:
• Physician referral form
• Medical insurance card
• Your photo ID
• A list of your medications – including dosages
What if I am getting therapy as a result of a worker’s compensation claim?
You will need to bring your claim information, including claim number.
What time should I arrive?
Please arrive 15 minutes early on your first visit to fill out and verify any necessary paperwork.
What do I wear?
We recommend comfortable clothing that allows for easy movement – such as sweatpants or shorts, a T-shirt and sneakers.
